All absences MUST be reported via this form on the first day of absence by 8.30am. Any absences not reported will be recorded as unauthorised & may result in a home visit from our Senior Leadership Team.

If your child requires medication during the school day, you must complete the 'Medication Request Form'. All medication must be handed into a member of the school office team. We cannot give any medication without a completed form.

All medication must be in the original packaging.
